My thoughts were that the production of such an item would be limited due to the cutting procedures for mass production of wood...but then again, switchplates exist, and those have rectangular holes. There simply could be any demand for such a thing.
So...
you can get a blank and learn how to use a scroll saw, and then maybe sell enough plates to pay for your saw,
buy a black plastic cover if there is one,
buy a white one and cover it with wood-grain contact paper (oh so John Waters!),
try calling
